maybe you should also try with another OS, like XENIX.

I had strange problems when I formatted for LOS then XENIX (in pure emulation). Also an older LOS release could be a good option because it really format and does not try to coexist with MW.

Also, I remember someone mentioned an alternative lisa test disk in this forum.
