if I understand that correctly: You tested the probably corrupted I/O Board with a replaced COPS chip and it worked? So the solution will be, the COPS is weak, leave it replaced and it will work.

By the way, the main reason is: Sometimes the contacts of the Lisa- case are bad due to corrosion or they are simply dearranged and have no contact if a board is plugged in. Check that carefully and try your I/= Board again if it's not the COPS-error.

The second error could be a weak power supply, which could mount a board so or so, but when started with another board (which consumes a little bit more current due to perhaps leak caps) it will fail to get in overcurrent position and your Lisa won't boot.
