I'm not shure, but only reading the name TOM I believe it could be the image of that disk here, which one pair of them I donated to a friend of mine in the states... could be !!! I'm not shure about that :-)
I bought a few of them (together with several Lisas, and the complete development inventory with furniture: chairs, desks, plants etc. = the complete room!!!) from an auction sold-out mid 80ies when closing a hifi- and computer- and consumer-electronics factory near my hometown.
