A 5MB ProFile's loud noise is typically due to the static brush on the spindle. Changing its position slightly so a different spot on the brush is touching the spindle often fixes it:

Changing the bearing requires more or less complete disassembly, so you don't want to approach that task unless you have no other choice.

To low level format a 5MB ProFile, you need an Apple /// (that works) with a ProFile interface card, and the software/firmware (some are available from VintageMicros.com)
