I'll see what I can do---I don't own a scanner.
While it doesn't help with some of the other notches and holes unique to Twiggys, you can do a pretty good job at getting a template for the other hole for the r/w heads by taking another 5.25" diskette jacket, rotating it 180 degrees, and lining up the edges.
For the other three necessary cuts, I don't think you need 300 DPI precision (and I doubt you can achieve it anyway with an X-acto; I know I certainly didn't). Instead, I'd just print out this image:

to the proper scale, superimpose it on your floppy, and cut to match. Note that the image does have some shadows that make it hard to see edges, but I think you could still get away with it.
