error 10726 when trying to boot from 5mb Profile

i just did a fresh LOS 3.1 install (went fine) but when i try to boot from the Profile i get an error 10726 and it won't boot. I can boot from LOS floppy disk 1 no problem. i can re-install LOS over and over with no issues. If i chose to repair disk it repairs about 50% of the time the other 50% it gives the error "Lisa cannot repair the attached disk".

any ideas?
