I don't know/recall what specifically can cause that behaviour, but the things that come to my mind as suspects are:

1. is the floppy disk unlocked? (The installer needs to write the serial number onto the disk)
2. enough memory? (you need 1MB or more for LOS)
3. is the widget completely working... will it boot into MW XL 3.0? Even if it does, it may have bad sectors. What messages are shown before it fails? (Has it started to initialize the drive when it fails?)
4. is the floppy drive working ok? (old grease removed and re-lubricated)
5. Poor/dirty contacts cause all sorts of Lisa problems. If all else fails, try cleaning the edge connectors for the CPU, I/O, and mother boards with electronic contact cleaner.

Installing the OS will initialize the hard drive by writing zeros to all the sectors; you don't need to do anything to the hard drive before the installation, but it does need to be a good drive.
