This is an absolutely stock Lisa 2/5 with H/A8 roms. It turns out to be an apparent electrical connectivity issue. The unit had some serious corrosion from NiCad battery leakage. I thought I had cleaned it up sufficiently with PCB flux remover and contact deoxidizer, but after additionally soaking overnight in dilute vinegar, it sprung back to life.

Very odd that MacWorksXL had no issues with the drive. I'm going to guess that the line carrying the parallel bus parity bit was not making connection and that the MacWorks ProFile device driver never checks it.
