I am sorry you are having issues with this board from Vintage Micros.  Normally these boards are very reliable, you should not have deal with this much trouble to get it going.

I wanted to wrap up this thread before I forgot:

The SCSI card is now working and tests with SCSI probe perfectly.

The problem was the ROM.

I got a QuickBoot ROM from Sigma Systems and it fixed the problem.

One last follow-up question: Does using the QuickBoot ROM have any issues or drawbacks compared to the original ROM?
