I have done a little more testing:

As Tom mentioned, I think these could all be symptoms of bad power, bad drive or bad drive controller board. And as jerry mentioned, this setup was working (as is) a few years back, so It is very unlikely to be a ROM version problem, or mismatched parts ...will investigate further.
